Summary of select health care and related provisions in House-passed Reconciliation Bill (HR 1) and Senate Amendment signed into law

On July 4, President Trump signed into law the HR 1, previously known as the One Big, Beautiful Bill Act, which includes policies impacting Medicaid and the Affordable Care Act (ACA) to help pay for the Tax Cuts & Jobs Act (TCJA) extensions. The table (attached below) provides an overview of the health care provisions included in the bill passed by the Senate on July 1 and by the House on July 3, as compared to the Finance Committee's June 16 proposed text and the House-passed One Big, Beautiful Bill Act (OBBBA, HR 1) from May 22.

Cost and savings projections for the Senate amendment reflect the latest Congressional Budget Office scores as of the time of publishing: the Amendment in the Nature of a Substitute to HR 1 posted on the website of the Senate Committee on the Budget on June 27, 2025. These scores do not reflect the final enacted bill.
